{"id":1583,"date":"2023-11-07T13:52:57","date_gmt":"2023-11-07T13:52:57","guid":{"rendered":"https:\/\/drmiltie.com\/?p=1583"},"modified":"2024-01-19T14:34:11","modified_gmt":"2024-01-19T14:34:11","slug":"new-blood-test-may-help-better-detect-early-stage-lung-cancer","status":"publish","type":"post","link":"https:\/\/drmiltie.com\/at-home-testing\/new-blood-test-may-help-better-detect-early-stage-lung-cancer\/","title":{"rendered":"New Blood Test May Help Better Detect Early Stage Lung Cancer"},"content":{"rendered":"\n<ul class=\"wp-block-list\">\n<li><strong>Lung cancer is the leading cause of cancer-related deaths globally.<\/strong><\/li>\n\n\n\n<li><strong>Most lung cancer cases are not diagnosed until the advanced stage, lowering a person\u2019s 5-year survival rate.&nbsp;<\/strong><\/li>\n\n\n\n<li><strong>Currently,\u00a0<em><u>the only screening available for lung cancer is a low-dose CT, which has some limitations.<\/u><\/em> A\u00a0<em><u>cancer genomics company says it has developed a blood test for the detection of early-stage lung cancer.\u00a0<\/u><\/em><\/strong><\/li>\n<\/ul>\n\n\n\n<p><strong><a href=\"https:\/\/www.medicalnewstoday.com\/articles\/323701\" target=\"_blank\" rel=\"noreferrer noopener\">Lung cancer<\/a>&nbsp;is the&nbsp;<a href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/lung-cancer\" target=\"_blank\" rel=\"noreferrer noopener\">leading cause<\/a>&nbsp;of cancer-caused death in the world. In 2020, there were about&nbsp;<a href=\"https:\/\/acsjournals.onlinelibrary.wiley.com\/doi\/10.3322\/caac.21660\" target=\"_blank\" rel=\"noreferrer noopener\">2 million diagnoses<\/a>&nbsp;and 1.8 million deaths due to lung cancer.&nbsp;<\/strong><\/p>\n\n\n\n<p><strong>The&nbsp;<a href=\"https:\/\/www.cancer.org\/cancer\/types\/lung-cancer\/detection-diagnosis-staging\/survival-rates.html\" target=\"_blank\" rel=\"noreferrer noopener\">5-year survival rate<\/a>&nbsp;for lung cancer depends on the type of cancer a person has, how early it is caught, and if the cancer has moved to other areas of the body. Lung cancer is more curable in its early stages.&nbsp;<\/strong><\/p>\n\n\n\n<p><strong><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7950268\/\" target=\"_blank\" rel=\"noreferrer noopener\">Previous research<\/a>&nbsp;has shown that most lung cancer cases are diagnosed at an advanced stage because many people do not have obvious symptoms early on.<\/strong><\/p>\n\n\n\n<p><strong><em><u>Currently, the only screening available for lung cancer is a low-dose\u00a0<a href=\"https:\/\/www.medicalnewstoday.com\/articles\/153201\" target=\"_blank\" rel=\"noreferrer noopener\">computed tomography (CT) scan<\/a>. However, CT scans can sometimes lead to\u00a0<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2777242\" target=\"_blank\" rel=\"noreferrer noopener\">false positives<\/a> or negatives.<\/u><\/em><\/strong><\/p>\n\n\n\n<p><strong>Now, a new way to find lung cancer at its early stages is in the works.<\/strong><\/p>\n\n\n\n<p><strong>At this year\u2019s European Society for Medical Oncology&nbsp;<a href=\"https:\/\/www.esmo.org\/meeting-calendar\/past-meetings\/esmo-congress-2023\" target=\"_blank\" rel=\"noreferrer noopener\">annual congress<\/a>,&nbsp;<a href=\"https:\/\/www.businesswire.com\/news\/home\/20231021966799\/en\/Analysis-from-TRACERx-Study-Reveals-More-Sensitive-and-Earlier-ctDNA-Detection-in-Lung-Cancer-Patients-by-Personalis-MRD-Technology\" target=\"_blank\" rel=\"noreferrer noopener\">initial findings<\/a>&nbsp;were presented for a new blood test for the detection of early-stage lung cancer.<\/strong><\/p>\n\n\n\n<p><strong>The research hasn\u2019t been published yet in a peer-reviewed journal.<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-the-lung-cancer-blood-test-works-nbsp\">How the lung cancer blood test works&nbsp;<\/h2>\n\n\n\n<p><strong>Developed by\u00a0<em><u>cancer genomics company Personalis Inc., the new test \u2014 called the\u00a0NeXT Personal ctDNA assay\u00a0\u2014 can detect small traces of\u00a0<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5217053\/\" target=\"_blank\" rel=\"noreferrer noopener\">circulating tumor DNA<\/a>\u00a0(ctDNA) in the blood, which is a marker for residual or recurrent cancer, said\u00a0<a href=\"https:\/\/www.personalis.com\/team\/\" target=\"_blank\" rel=\"noreferrer noopener\">Dr. Richard Chen<\/a>, chief medical officer and executive vice president of research and development at Personalis Inc.<\/u><\/em><\/strong><\/p>\n\n\n\n<p><strong>\u201cThe NeXT Personal technology leverages whole genome sequencing and advanced noise suppression with NeXT SENSE technology to identify an ultra-sensitive, unique genetic signature derived from a patient\u2019s tumor,\u201d Chen explained to&nbsp;GMHCN. \u201cThis unique signature is tracked in the patient\u2019s blood over time to find residual or recurrent cancer, achieving an industry-leading sensitivity of down to ~1 part per million of ctDNA.\u201d&nbsp;<\/strong><\/p>\n\n\n\n<p><strong>\u201cThis enhanced sensitivity offers the potential for earlier recurrence risk assessment and intervention, earlier detection, more precise monitoring, and substantial advancement in lung cancer care,\u201d he added.&nbsp;<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-identifying-recurrence-risks-prior-to-surgery\">Identifying recurrence risks prior to surgery<\/h2>\n\n\n\n<p><strong>According to&nbsp;<a href=\"https:\/\/www.crick.ac.uk\/research\/labs\/charles-swanton\" target=\"_blank\" rel=\"noreferrer noopener\">Dr. Charles Swanton<\/a>, the deputy clinical director of the Francis Crick Institute in London and principal investigator for the&nbsp;<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/37055640\/\" target=\"_blank\" rel=\"noreferrer noopener\">TRACERx study<\/a>, the main insight from the work with the NeXT Personal assay in TRACERx concerns disease stratification in early-stage lung cancer.&nbsp;<\/strong><\/p>\n\n\n\n<p><strong>\u201cWe have at present an imperfect system to understand which patients will typically have a poor clinical outcome following&nbsp;<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8345199\/\" target=\"_blank\" rel=\"noreferrer noopener\">surgical resection<\/a>&nbsp;of their primary tumor with curative intent, and therefore we aren\u2019t always able to direct additional supplemental therapy, called&nbsp;<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9149592\/\" target=\"_blank\" rel=\"noreferrer noopener\">adjuvant therapy<\/a>, to those who would benefit it most,\u201d Swanton explained to&nbsp;<em>GMHCN<\/em>. \u201cThis leads to under-treatment of some patients and over-treatment of others. Therefore, there is an urgent need for better ways of stratifying those patients who will do well after surgery and those who will not. This insight was the backdrop to our study.\u201d&nbsp;<\/strong><\/p>\n\n\n\n<p><strong>Classifying patients into high and low risk with high fidelity is important, Swanton said, because it opens the door to \u201dpersonalized\u201d trials of adjuvant therapy.&nbsp;<\/strong><\/p>\n\n\n\n<p><strong>\u201cBeing able to withhold treatment from those unlikely to benefit but give it within a population of patients who would benefit could be very impactful,\u201d he noted. \u201cIn terms of detection of recurrence, we know that in patients with evidence of ctDNA in the blood following surgery, it is possible to detect relapse almost one year ahead of routine clinical surveillance. As with the pre-operative data, this opens the door to a more personalized treatment approach, where aggressive treatment can be deployed in the early-metastatic setting.\u201d&nbsp;<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-lung-cancer-blood-test-in-clinical-trials\">Lung cancer blood test in clinical trials<\/h2>\n\n\n\n<p><strong>Chen said the NeXT Personal test is currently available for clinical testing as part of an early access program at Personalis.<\/strong><\/p>\n\n\n\n<p><strong>\u201cEarly-stage lung cancer recurs at a very high rate with approximately 20 to 40 percent of patients eventually recurring depending on stage,\u201d he said. \u201cCurrent methods of recurrence detection like imaging or other blood tests can have limited sensitivity. Finding more sensitive ways to detect residual cancer after treatment or recurrent cancer can open new opportunities to treat or escalate treatment for lung cancer patients earlier.\u201d<\/strong><\/p>\n\n\n\n<p><strong>Swanton added the next research steps for this new blood test would be to have findings validated in prospective cohorts.&nbsp;<\/strong><\/p>\n\n\n\n<p><strong>\u201cThis will open the door to routine clinical use and the definition of specific cohorts in which an ultra-sensitive assay can be utilized,\u201d he said.&nbsp;<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-importance-of-recurrence-early-identification\">Importance of recurrence early identification<\/h2>\n\n\n\n<p><strong>After reviewing information about the new NeXT Personal ctDNA assay,&nbsp;<a href=\"https:\/\/doctors.baptisthealth.net\/provider\/John+Robert+Roberts\/2059046\" target=\"_blank\" rel=\"noreferrer noopener\">Dr. John Roberts<\/a>, a thoracic surgeon with Lynn Cancer Institute, part of Baptist Health South Florida, at Boca Raton Regional Hospital, told&nbsp;<em>GMHCN<\/em>&nbsp;that as CT scanning for lung cancer has been shown to decrease mortality from lung cancer more than any medical treatment, so an accurate blood test should have a similar impact.<\/strong><\/p>\n\n\n\n<p><strong>\u201cAll of our blood testing for lung cancer only identifies advanced cancer,\u201d he explained. \u201cAny test that could be used as a screening test for early cancer would be a dramatic advance since our only screen for lung cancer now is a CT scan.\u201d<\/strong><\/p>\n\n\n\n<p><strong>\u201cMost patients who develop recurrence after treatment for lung cancer are incurable by the time their cancer is discovered,\u201d Roberts added. \u201cAny test that results in an early identification of recurrence would be important.\u201d<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-scaling-up-screening-efforts\">Scaling up screening efforts<\/h2>\n\n\n\n<p><strong>GMHCN&nbsp;<\/strong><strong>also spoke with&nbsp;<a href=\"https:\/\/www.saintjohnscancer.org\/about-us\/people\/sara-belton-ph-d-rn\/\" target=\"_blank\" rel=\"noreferrer noopener\">Sara Belton<\/a>, PhD, a nurse navigator for Providence Saint John\u2019s Health Center\u2019s Lung Screening Program at the Cardiothoracic Outpatient Clinic in California, about this study.<\/strong><\/p>\n\n\n\n<p><strong>As a nurse navigator whose work focuses on lung cancer screening, she said that any tools that can help clinicians in the diagnosis and long-term management of lung cancer are most welcome.&nbsp;<\/strong><\/p>\n\n\n\n<p><strong>\u201cThere are several research consortia working on developing a blood test for lung cancer genome detection and it is seen by many in the field as the \u2018Holy Grail\u2019 for lung cancer diagnosis,\u201d Belton said. \u201cResearch success in this area will help clinicians by providing an easier-to-access and potentially more reliable lung cancer screening modality than the current standard, a CT imaging scan, which needs good imaging visualization and clinical interpretation to be most effective. I welcome this news and look forward to hearing more about their results over time.\u201d&nbsp;<\/strong><\/p>\n\n\n\n<p><strong>She added that having a low-cost, easily accessible blood test that could help screen people for lung cancer will help medical professionals scale up screening efforts, as is currently low in the United States.&nbsp;<\/strong><\/p>\n\n\n\n<p><strong>\u201cRight now, clinical guidelines for&nbsp;<a href=\"https:\/\/www.uspreventiveservicestaskforce.org\/uspstf\/recommendation\/lung-cancer-screening\" target=\"_blank\" rel=\"noreferrer noopener\">lung cancer screening<\/a>&nbsp;recommend that only current or former smokers with at least a 20-pack year smoking history and who are over age 50 be screened annually for lung cancer,\u201d Belton noted. \u201cHowever, up to 10 percent of people who develop lung cancer do not have a smoking history and they are not included in these guidelines. These people are often the ones whose lung cancer is found too late for optimal treatment and best outcomes.\u201d<\/strong><\/p>\n\n\n\n<p><strong>\u201cHaving a blood test that could be more broadly administered would help us clinicians find these at-risk non-smokers, and would allow us to intervene and treat them early enough to have good health outcomes and even save them from dying of lung cancer,\u201d she added. \u201cThis would be a huge advancement in lung cancer screening and lung cancer care and I am very hopeful that such a blood test will become commercially available in the next few years.\u201d<\/strong><\/p>\n\n\n\n<p><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Lung cancer&nbsp;is the&nbsp;leading cause&nbsp;of cancer-caused death in the world.
