Arizona reimbursement source review completed August 31, 2026. AHCCCS states that both its Contractor and fee-for-service programs cover synchronous and asynchronous remote patient monitoring. Equipment and supplies are covered only when the underlying service is an AHCCCS-covered service eligible for remote monitoring and the equipment or supplies are themselves covered items. The current AHCCCS telehealth code set controls billable RPM codes, modifiers and place-of-service requirements, so code-level status and plan-specific rules should be checked before billing.

CMS finalized CY 2026 Physician Fee Schedule policy effective January 1, 2026. CMS currently describes Medicare RPM as connected-device monitoring for acute or chronic conditions, with digitally uploaded physiologic data and program-specific service requirements. The 2026 RPM code set added CPT 99445 and 99470 for lower-day and lower-time monitoring scenarios.

This Medicare baseline is national. It does not establish Arizona Medicaid or MCO coverage, payment, provider eligibility, or billing rules.
