Rhode Island Medicaid & Medicare Reimbursement for RPM, CCM & TCM
Use this page as a structured planning reference for Rhode Island. Medicare code families provide a national baseline; Medicaid program, managed-care, provider-type, and setting rules require state and plan-level verification.
Rhode Island reimbursement source review completed August 31, 2026. Rhode Island Medicaid maintains current telemedicine billing guidance, managed-care contract requirements, and a mandatory minimum telemedicine code resource for Medicaid managed care. Current contract language recognizes remote patient monitoring devices within the broader telemedicine framework, but the reviewed first-party material does not establish blanket 2026 payment of the full Medicare RPM CPT family or one universal rate across fee-for-service and managed care. Organizations should verify the current EOHHS fee schedule, mandatory minimum code list, provider eligibility, documentation and authorization rules, and the applicable MCO contract before modeling RPM reimbursement.
Effective/source context: Rhode Island EOHHS provider guidance and managed-care contract resources are current through 2026; Telemedicine Billing Guidance is effective September 1, 2025 and 2026 MCO amendments remain posted. What to verify in Rhode Island
Build the reimbursement picture in layers
Medicare baseline
Confirm the applicable physician fee schedule, code descriptors, supervision rules, device/data requirements, time thresholds, and any annual coding changes.
Rhode Island Medicaid program
Review the current fee schedule, provider manual, telehealth or remote-monitoring guidance, eligible provider types, places of service, and billing limitations.
Managed care
Do not assume state-level policy equals MCO payment. Verify plan contracts, prior authorization, care-management benefits, alternative payment methods, and value-based arrangements separately.
Revenue model
Model payer mix, eligible patient volume, realistic utilization, staffing, device/platform expense, and contract-specific reimbursement only after the policy assumptions are verified.
Verified Medicare baseline
CY 2026 national policy, checked August 30, 2026
CMS finalized CY 2026 Physician Fee Schedule policy effective January 1, 2026. CMS currently describes Medicare RPM as connected-device monitoring for acute or chronic conditions, with digitally uploaded physiologic data and program-specific service requirements. The 2026 RPM code set added CPT 99445 and 99470 for lower-day and lower-time monitoring scenarios.
This Medicare baseline is national. It does not establish Rhode Island Medicaid or MCO coverage, payment, provider eligibility, or billing rules.
