Wyoming reimbursement source review completed August 31, 2026. Wyoming Department of Health expressly corrected a federal summary that had labeled Wyoming as not reimbursing remote monitoring and states that Wyoming Medicaid reimburses CPT 99453, 99454, 99457 and 99458. The same state application also identifies additional HCBS waiver remote-monitoring service codes. Because the statement predates the 2026 additions 99445 and 99470 and does not provide current payment amounts, organizations should verify the current Wyoming Medicaid fee schedule, provider manual, service limits and any program-specific authorization rules before billing.

CMS finalized CY 2026 Physician Fee Schedule policy effective January 1, 2026. CMS currently describes Medicare RPM as connected-device monitoring for acute or chronic conditions, with digitally uploaded physiologic data and program-specific service requirements. The 2026 RPM code set added CPT 99445 and 99470 for lower-day and lower-time monitoring scenarios.

This Medicare baseline is national. It does not establish Wyoming Medicaid or MCO coverage, payment, provider eligibility, or billing rules.
