Chronic Care Management Services

Medicare has provided separate payment for qualifying chronic care management (CCM) services since 2015. CCM generally involves ongoing, non-face-to-face care coordination for patients with two or more qualifying chronic conditions. CCM is distinct from remote patient monitoring (RPM), which involves transmitting health data from connected devices.
Consult the CMS Chronic Care Management Services booklet for current CCM requirements and billing information. For device-based monitoring, see CMS guidance on Remote Patient Monitoring. Historical context: this article was first published May 5, 2015; Medicare billing requirements may have changed since its original publication.
