Kansas reimbursement source review completed September 5, 2026. Kansas now uses the live KMAP fee-schedule portal as the primary operational billing source rather than the Rural Health Transformation budget narrative. KMAP states that providers can review current and historical fee schedules there, but also warns that fee-schedule information does not guarantee coverage or payment and that provider manuals must be consulted for coverage, program limitations, provider type and specialty, and beneficiary-specific rules. Kansas' 2026 Rural Health Transformation investment remains useful evidence of an active remote-monitoring delivery pathway, but it is not a Medicaid billing authority. Before modeling RPM revenue, verify each RPM code in the current KMAP fee schedule and applicable provider manual, together with provider-type eligibility, authorization, documentation, beneficiary coverage and KanCare managed-care requirements.

Verified Medicare baseline

CY 2026 national policy, checked August 30, 2026

CMS finalized CY 2026 Physician Fee Schedule policy effective January 1, 2026. CMS currently describes Medicare RPM as connected-device monitoring for acute or chronic conditions, with digitally uploaded physiologic data and program-specific service requirements. The 2026 RPM code set added CPT 99445 and 99470 for lower-day and lower-time monitoring scenarios.

This Medicare baseline is national. It does not establish Kansas Medicaid or MCO coverage, payment, provider eligibility, or billing rules.

Quarterly PFS timing safeguard

As of September 23, 2026, the service-date-appropriate 2026 Medicare release is July 2026 (RVU26C / PFREV26C). Use the PFS release applicable to the actual date of service, not simply the newest file CMS has posted.

The October 2026 RVU26D / PFREV26D files may already be available, but they should not be used for dates of service before October 1, 2026. CMS also maintains separate QP and non-QP files for 2026 because different conversion factors can apply.
