The state provider notice added CPT 99453, 99454, 99457, 99458 and 99091 effective October 1, 2023 and states that those listed codes did not require service authorization at issuance. Because the code notice predates the 2026 CPT additions, organizations should verify the current fee schedule, any newer RPM codes, quantity limits, provider eligibility, and the applicable Medicaid managed-care plan before billing.

CMS finalized CY 2026 Physician Fee Schedule policy effective January 1, 2026. CMS currently describes Medicare RPM as connected-device monitoring for acute or chronic conditions, with digitally uploaded physiologic data and program-specific service requirements. The 2026 RPM code set added CPT 99445 and 99470 for lower-day and lower-time monitoring scenarios.
