TennCare states that medical services are delivered through managed-care organizations and that each MCO creates its own provider contracts, maintains its own fee schedules and processes its own claims. The current first-party public material reviewed for this audit did not establish one statewide TennCare RPM fee schedule or universal payment of the full Medicare RPM CPT family. Organizations should verify the member's MCO, current plan fee schedule and medical policy, provider contract, authorization requirements, documentation rules, and any TennCare program guidance before modeling RPM reimbursement.

Verified Medicare baseline

CMS finalized CY 2026 Physician Fee Schedule policy effective January 1, 2026. CMS currently describes Medicare RPM as connected-device monitoring for acute or chronic conditions, with digitally uploaded physiologic data and program-specific service requirements. The 2026 RPM code set added CPT 99445 and 99470 for lower-day and lower-time monitoring scenarios.

This Medicare baseline is national. It does not establish Tennessee Medicaid or MCO coverage, payment, provider eligibility, or billing rules.
